Sub: Rectification of the spelling of the name on H.S admit card
Respected Sir,
Humbly and with due submission, I, Ritika Singh(you have to write your name), an examinee of the ensuing H.S from Behala Girls’ High School( you have to write your school’s name) under your Council (bearing Reg. No. – 4757( your Registration no) of 2019( the year of registration) and Roll No. – N321-730(your Roll No)), beg to state that there is a minor mistake of spelling of my name in the admit card. The proper spelling of my name as per my birth certificate is – RITIKA SINGH, but my admit card shows it as – RITIKA SINHA (Don’t forget to write these two names in capital letters). This may result in serious problems in the examination hall and even in my future career. I have enclosed herewith the photocopies of my birth certificate, H.S admit card, Registration and the forwarding letter of my institution for your kind consideration and prompt rectification.
Hope the matter must be taken sincerely and a fresh admit card will be issued in my favour as early as possible.
Yours Faithfully
Ritika Singh
Candidate of H.S – 2021
(the year of your exam)
(Your School’s name)
Thanking you,
